At the Appointment

Lab Tests
- The exam requires testing for tuberculosis (TB) and syphilis for most adults and that is done with blood work.
- If you have been treated for TB in the past, we will need those records and you will then need to have a chest x-ray completed. This can be completed prior to your visit or can be ordered on the day of your visit.
- If your bloodwork for TB is positive, you will then need a chest x-ray prior to completion of the exam paperwork.
- Those between ages 18-24 also need a urine test for gonorrhea.
- These tests will be ordered on the day of your visit and should result within 3-5 days at the lab.
Required Vaccines
Vaccines may be necessary to complete the form and we will determine that at the visit.
- See current vaccine requirements (CDC)
- Please note these requirements may be updated from time to time.
- Vaccine series that are required by age must at least be started at the time of form completion.
